This session will describe how the integration of a patient navigator program and a Management Information System led to increased rates across the HIV continuum of care in a single-provider clinic in El Paso, Texas.
Learning Objectives
- Assess the importance of integrating and systematizing data analysis to clinical workflows to bring visibility to potential gaps in care.
- Describe how the patient navigator program as well as the Management Information System supports the national HIV/AIDS Strategy primary goals of increasing access to care and optimizing health outcomes of people with HIV and reduce HIV-related health disparities and health inequities.
- Identify multi-tiered, data-driven strategies that can be used to address linkage and retention in care in border communities.
