Steps to Care Tools on HIV Care Engagement
The August 25 issue of CDC's Daily Digest Bulletin featured a handful of resources that we're going to parse out to you this week in the daily feed. First up: a STEPS to Care Toolkit with client- and provider-focused tools to support implementation of three evidence-informed strategies to engage and retain people in care: Patient Navigation, Care Team Coordination, and HIV Self-Management.
Everything is online, so take a few minutes to orient yourself to what's available as there are pages that have more of an overview purpose while the Dashboard presents four steps to care with details on the specific tools to use, like assessing agency readiness, planning, tracking clients, and more. Steps to Care is part of CDC's Effective Interventions collection and is based upon work out of New York City's Ryan White Care Coordination program.
