This document was prepared by participants in the In the HRSA/SPNS Workforce Initiative focusing on HIV practice transformation (2014 – 2018). Over half of the 15 demonstration sites designed and began integrating into their practice various collaborative Care Teams, which were to meet the sites’ unique patient population needs and organizational contexts.
