Upgrading to CAREWare 6: Technical Assistance Request Form

Please provide the information requested in this form so that we can assist you and your agency most effectively in your upgrade to CAREWare 6. This form should be completed by an IT or other staff member that is responsible for overseeing your organization’s CAREWare system. 

A staff member from the CAREWare helpdesk will contact you within three business days to review the information and to schedule a call.

PLEASE NOTE: If you are a provider that is connected to a central server that is managed by the grant recipient, the TA request should come directly from the recipient or entity overseeing the network and not from the individual providers.

4. Is CAREWare Internet-facing or are there users connecting from outside of the network?
5. If yes to #4, has the TLS certificate been imported to the CAREWare business tier server?
6. Is SQL Server Management Studio installed, and are you able to connect to the CAREWare database?

9. Is CAREWare being installed on a new server/PC?
10. If yes to #9, have Windows updates been completed after installing SQL Server?
11. Are users connecting to CAREWare through Citrix or VPN?
